IMPORTANT:

• Fill out your address and contact information as accurately as possible, because all communications between you and the office will depend on this data.

• We do not take insurance. We are a direct-pay practice.

• A valid credit card number is required to enroll which fmust be entered to COMPLETE the enrollment.

• Patients and clients are not required to pay for services or medications via credit card, you can pay with an HSA/FSA card, personal check, even cash.

• You must be enrolled to schedule an appointment with the doctor or health coach or IV nurse.

All your health information is protected and private and will not be shared with anyone at anytime, with the following exceptions:

1.
